The last week or two I have seen a sharp uptick in the number of aimbot reports come in. Most often, they involve a spy using either the enforcer or the ambassador and a dead ringer. I try to take such accusations seriously, so I have spectated these players and I have seen a few I would swear

I myself haven't experienced an aimbotter for a few months, not since I installed SMAC (SourceMod Anti-Cheat) and enabled aimbot protection in it. It does give a few false positives, but it has maybe banned one innocent player per 20 confirmed aimbotters. Which I find acceptable. -Original

This is where votekicks/bans work well. If I cannot tell someone is hacking but I've gotten enough reputable complaints then I will just start a banvote, but only ban for 2 or 3 days. If they ARE aimbotting then they probably will just assume they've been banned for ever and won't be coming back.

I'm still seeing that tf2 items line failure to load etc on a server restart...and again I've seen several if not half a server of mine have clients crashing which to me says its something from the server. This hasn't happened until the recent big Xmas updatesomething is causing these issues
