Ok, I have access at "work" on a Compaq nx9420, for a few days.

Not sure if helpful, but I had similar issue - machine would not boot if
(only) using external monitors. My computer is a Dell Latitude 5590; it
had previously booted, upon power up, with the machine closed, thus
using only external monitors.
In order to diagnose, I started to roll back updates,
I verified that these two patches landed in the current 4.19 kernel of
disco (in release pocket now) - hence changing ticket to Fix Released.

I'm unable to reboot the faulty server to try to obtain any information
on the crash, but creating a brand new instance of 18.04 LTS on Hyper-V
2012 R2 boots quite happily on 4.0.15-45.
I've also noticed that I haven't installed the azure tuned kernel
package linux-azure.
