I think I know the problem. Those entries that failed to work had space, comma, space, comma, then the county name and state name. That meant that USA would have been one field too many, so it wasn't added. I am afraid if I use "find and replace" to replace the two space + commas, I will mess some

Check the ones that did not work. Were they missing commas to make the state names the third level of government. The tool looks for places with town, county, state with a comma for missing levels so if you have town, state the tool will not recognize the state as a state name. A place with the for
