Thanks! this looks what I need. The only problem is that I am not familiar
with customization and this does not look like a "\bind" line to put in a
mac.bind or math.bind - did I miss anything? where should I put this
command-alternatives break-paragraph;math-extern maxima  ?

Keceao, and of course you need to activate also "Sectioned Bibliography"

Numbering of Theorems, etc.

I use LyX 2.2.1 (currently) on Windows 10.

I have a rather long document (some 600 pages, with 9 chapters + several 
appendices), and want to number Exercises, Problems, Examples, Theorems, etc. 
*independently* and numbered *by the chapter* (or appendix) they appear in. 
Thus, exercises in chapter 6 should be numbered Exercise 6.1, Exercise 6.2, 
etc., while examples in chapter 6 should be numbered Example 6.1, Example 6.2, 
etc.

Currently, this doesn’t work. My set-up is:

· Document class: Book (Standard Class)

· Modules: Theorems (AMS, Numbered by Type)

What else do I need to set?

The mentioned documentation is an online version of the built-in Math manual 
help.
The procedure of binding an arbitrary key to a command (sequence) is part of
Appendix C of the users guide: C-2.2.1 Editing Shortcuts.

I assume the problem is that you are not getting the "numbered by
chapter" bit? If so, then maybe the easiest thing to do is use the
internal LaTeX command \@addtoreset in the preamble, thus:
\@addtoreset{theorem}{chapter}
Now the theorem counter gets reset whenever the chapter counter is reset.

You'll need to do the same thing for any other counters you are using,
and see the theorems-ams-bytype.inc file for the names of the various
counters.

There is no error in compiling Your code and it really works.
But there are some problems when I try to use your settings in the SVMONO 
template that is provided by Springer Publisher.
I am trying to figure out what is wrong.

This happened when exporting to html.
Something has changed.
Yes, there was some work on the dashes and double dashes. However 
this was

meant to be an improvement.

Depending on how the no-break-hyphens got into your document, your
problem may turn out to be an actual improvement (uncover a hithero
hidden problem) or an unwanted side-effect.


An export to Lyx html does give a correct result.

So using this route is a possible workaround/solution.

 Yes, that is a work around.  Also we could use an html editor, 
probably Bluefish
to correct the errors in the document. So we are not up against a 
brick wall.

  Sadly, using LyXHTML is not a workaround, the images are held in a 
separate
file and get lost when we convert to odt for our translators.  So the 
only present
solution is a correction with an html editor after exporting with HTML.  
I have looked at
an intermediate step using LateX but the short double dash is likely to 
be converted

into an 'en' dash.
