Windows or Linux? I am working in Windows, and if that happens, I just drag
the windows over to my primary screen or whatever screen I want to work on
and the next time I ask the windows to pop up they land right where I left
them :)

This may be wide off the mark, but my untrained eyes see a minor
variation of Aldus [1] [2]. Could someone confirm this?

footnote after the respective author. I.e.,

A suggestion:
You can try putting everything, including the "\author" command, inside
the ERT TeX-code box, and not use built it "\author" command of the LyX.
