Is there any debug code that we can put in that would allow users who
experience the issue to report back to us some information that would
help us narrow down the issue?

Or if it is possibly a Qt bug, then just hope that Qt 5.6 improves
things?

Will do - but do you know if this is a cross-platform issue or a 
windows-specific one? I haven't had any luck pinpointing the culprit (or how to 
replicate this), and it will be helpful to know if this is platform specific.

I think that most of the reports we've seen have been Windows-specific.


I know of no reports that aren't on Windows.

My fear is that it is really a Qt bug.

This happens to me (with all versions of lyx that I have been using so 
far)  when I leave the file in lyx open for too long (for days, for 
example). I resolve it by simply shutting it off and then restarting. 
It is not a serious problem.
