As requested, I've attached a sample LyX file that uses the exam layout. In this file you can see some of the tricks I used to get around a few of the shortcomings of exam.cls. I now use the examdesign.cls with ordinary LaTeX because it seems better for my own needs. --Jason Waskiewicz
