Have you checked use single letter navigation under Quick Nav settings 
in VO Utility?

Someone might have other suggestions but assuming you have done the 
usual, that is repaired disk permissions, , done the Pram reset and so 
on the I personally would be looking to re-install the OS if Voiceover 
was behaving so strangely despite multiple reboots.
Nowadays I set this up overnight and wake up in the morning to a 
re-installed OS so it seems pretty straightforward to me and is usually 
less effort than trying to disentangle problems.  A reinstall should 
return you to the same settings, apps and documents but with a better 
working Mac OS, including Voiceover. If a re-install does not work you 
may have to look at a clean install. Make sure you have a Time Machine 
backup before doing anything.


Let us know if you need help with the reinstall process.
