Hi Gary. Just an FYI, this has been around ever since I first used the mac back in 2012. Nothing new here, but definitely overlooked since it's buried within the VoiceOver Utility. Also, it is probably used less now caps lock can be used as the VoiceOver modifier since OS X 10.11 Yosemite.

Hi Don't know, but do know single user mode is not accessible because VoiceOver is not loaded in this mode. There might be a way in recovery mode to manage a locked account. For example in Terminal you can type resetpassword press return and you can reset the password for the local account. >
