OK. Try resetting your VoiceOver preferences in the VoiceOver Utility to factory defaults. If you have customized them, you will have to reset them manually, but I’ve discovered that doing this has really made a difference to curing a lot of the little VO problems I was initially having.

I can confirm this and I've written to the list about this before. Hotspots just don't stick at all. I had reported it in the beta testing and I've reported it since upgrading to El Capitan. Please keep on reporting it to Apple Accessibility. It's not just on hotspot 22, it's any hotspot I
