What 's the deal with people putting a long list of irrelevant
keywords in their craigslist posting, so that it comes up on searches
that have nothing to do with the item for sale?

This is called exposure, done to ensure that as many people as possible
look at the ad in the hope that someone will buy the crap you are
selling, even though they didn't want it.
For example if you where to sell a Merc, it would pay to put in keywords
that others may be looking for like,

I believe that if you check the only search titles box, the other
stuff/keywords are not included into your Craigslist search

Crudely is right. I searched for saab and got an ad for Lexus
wheels. Waste of my time (and the seller's, too, if I were as dumb as
