Prasugrel is a platelet inhibitor. Warfarin is a vitamin K-dependent clotting factor inhibitor (II, VII, IX, X). Xarelto and other doacs inhibit specific clotting factors (eg Xa and II). All of these factors and platelets play roles in the cascade leading to blood clot formation. Yes, it is MUCH more expensive. It is about $600 for a 30 day supply. With my insurance it was running around $180 per month. I signed up for some program from the manufacturer and was able to get it for $10 a month. In any case, after a little google searching, Xarelto is not effective for patients with whatever syndrome is that I have. That would explain why the doctor is switching my medication. [MBZ] OT: broken house window
So one of the windows in the bathroom got hit with a rock or hailstone - could have been either - and the tempered glass shattered but is still in place. We kind of like the effect actually, and would like to preserve it as is. Any ideas of how to do that? adhesive film of some sort? Glue of some kind? built 12/1973, so no CAT. California car. I am third owner. Records from 1987 on, when second owner purchased. I got it in 2012 from the son of the decease owner. It had 203k on the clock then, it has 215k-ish now. Serviced by local indy who sorted out D-jet issues and installed fresh injectors in 2017. He had done the fuel pump a few years before. I collected all the things that I thought it may need or did want fixed, like pass seat cushions, replacement for AC compressor, … amounting to a pick up load of extra/spare/replacement for the car. Updates were limited by cash flow, so that is why I have stuff in boxes. Currently sporting Euro lights. I have the DOT lamps in a box. Replacement lenses for the rear lights in case originals get hurt. Around six or seven large Rubbermaid tubs of R107 things. Silber Grun body, Dunkel Grun top and hubcaps. Green leather seats (soft without cracks) and interior. The center console was replaced because prior owner had butchered it to install Sony radio and in car Cell. Brown soft top that works but is getting old, driver side small window has a tear in plastic. Got minimal usage in 2019, as I was out half the year. No usage in 2020 (on battery tender). Got some miles on the clock in 2021, but sitting has made it need a tune up to put it back into full service. Did get oil and filter change around 100 miles ago. Been using non alcohol fuel since 2015 because it is less volatile and reduces known hot engine start issues. Trigger points replaced with rebuilt, MAP is good, Djet brain was replaced. There is a spare in the boxes. Transmission was last serviced 2017 and only 2500 miles since. Cooling system rebuilt. Been garage kept most of its life. No accidents while I owned it. Some evidence of prior panel replacement. Tires are good, alignment done in 2017 at 211k miles. Brakes are good, and I have replacement rotors and pads for when the current things age out. This is NOT BaT car, but a reliable rolling Preservation Class award car (2x). It does need the cross over pipe section of exhaust replaced, from there back is new since 2015. Headers and seals replaced around that time. Does 85mph for extended (200 miles) periods with ease. Does not blow cold air, at all. Does have not original Becker Mexico that pulls in stations when it wants. Power antenna does work, but does not pull in signal well, so has been unpowered. I collected all manner of original dealer literature (sales, paint brochure, and on and on) Even have matchbox example 107 in green paint in the glove box. The CLK certainly "looks" like a coupe version of the E class, but I don't know if they share any common parts. If they do, the E class W210 and W211 are very robust, if mundane cars. There is more wood that came over the border in PNW than is harvested locally. Most of the mills in WA closed down ages ago, and the lumber companies have been pulling back from harvests. At least that was the case in 2019. Not being in residence the past year, I have not been witness to current mills. The processor near my mother’s home was bare of completed/dried/cut lumber and the yard was minus the normal inventory of logs for this time of year. The scrap market in this neck of the woods is a monopoly of Schnitzer which sells crushed cars to the CCP. I am not seeing the standard traffic of trucks headed out with dead/squished cars that I used to. Not been hitting the PnP, as I have nothing I need to get there. Also, they have clamped down on how many people are allowed in the yards. Coumadin and Xarelto have different mechanisms of action I believe, and while I don't know much about the actual condition you have, I would assume coumadin is more effective in that situation that other anti-coagulants. Same reason the blood clots that are associated with the J vaccine should not be treated with heparin, which is the normal "first line" action for clotting issues. Not sure of the mechanism of the formation of that particular type of clot, but it occurs along with suppressed platelet count, and heparin works by preventing platelet conglomeration if I remember my med tech training from forty years ago. Good evening A friend is considering a classic convertible, either a Saab or MB. He was interested in a 2001 W208 (A208, I guess, based on the W202) CLK430 convertible. Or maybe a later 209. I suggested he consider a W124 cabrio. IMO more comfortable and more reliable, just need to be aware of the potential wiring nightmare. So, the question is which of the three options is best, and if it has to be a 208/209, which is the lesser evil? [MBZ] Blood thinners etc
As you may recall a while back I was in the hospital due to blood clots. They put me on Xarelto. It was still a mystery as to why this happened in the first place. My primary doctor sent me to a cancer doctor to figure it out. Went back to him for the 2nd time last week to run tests. Turns out I evidently have something called antiphospholid antibody syndrome. They are moving me from Xarelto to Coumadin for some reason which will require me to get blood tests weekly for a few weeks then monthly after. I guess I will ask but not sure why they would take me off the newer state of the art blood thinner that requires no testing to this older cheaper medication that does. The evidence for the DOACs like Xarelto for antiphopholipid antibody is not as strong as the evidence for warfarin. There is some suggestion that Xarelto etc may not work as well for arterial clots, though they both seem to work OK for venous clots. Anyway it has not been super well studied but until there is a good study showing comparable efficacy with both drugs in a large number of patients, the recommendation will likely continue to be for warfarin. At least that is my understanding. Peer-reviewed study in the American Journal of Therapeutics has identified a very effective drug for use against the Wuhan Red Death (aka COVID-19). Furthermore, results from numerous controlled prophylaxis trials report significantly reduced risks of contracting COVID-19 with the regular use of ivermectin. Finally, the many examples of ivermectin distribution campaigns leading to rapid population-wide decreases in morbidity and mortality indicate that an oral agent effective in all phases of COVID-19 has been identified. Exposure prophylaxis studies of ivermectin's ability to prevent transmission of COVID-19 Data are also now available showing large and statistically significant decreases in the transmission of COVID-19 among human subjects based on data from 3 randomized controlled trials (RCTs) and 5 observational controlled trials (OCTs) with 4 of the 8 (2 of them RCTs) published in peer-reviewed journals.40– Clinical studies on the efficacy of ivermectin in treating mildly ill outpatients Currently, 7 trials that include a total of more than 3000 patients with mild outpatient illness have been completed, a set composed of 7 RCTs and 4 case series.49–60 The largest, a double-blinded RCT by Mahmud49 was conducted in Dhaka, Bangladesh, and targeted 400 patients with 363 patients completing the study. In this study, as in many other of the clinical studies to be reviewed, either a tetracycline (doxycycline) or macrolide antibiotic (azithromycin) was included as part of the treatment. The importance of including antibiotics such as doxycycline or azithromycin is unclear; however, both tetracycline and macrolide antibiotics have recognized anti-inflammatory, immunomodulatory, and even antiviral effects (58–61). Although the posted data from this study does not specify the amount of mildly ill outpatients versus hospitalized patients treated, important clinical outcomes were profoundly affected, with increased rates of early improvement (60.7% vs. 44.4% *P* < 0.03) and decreased rates of clinical deterioration (8.7% vs. 17.8%, *P* < 0.02). Given that mildly ill outpatients mainly comprised the study cohort, only 2 deaths were observed (both in the control group). Clinical studies of the efficacy of ivermectin in hospitalized patients Studies of ivermectin among more severely ill hospitalized patients include 6 RCTs, 5 OCTs, and a database analysis study.45,51–53,63–70 The largest RCT in hospitalized patients was performed concurrent with the prophylaxis study reviewed above by Elgazzar et al.45 Four hundred patients were randomized among 4 treatment groups of 100 patients each. Groups 1 and 2 included mild/moderate illness patients alone, with group 1 treated with one dose 0.4 mg/kg ivermectin plus standard of care (SOC) and group 2 received hydroxychloroquine 400 mg twice on day 1 then 200 mg twice daily for 5 days plus standard of care. There was a statistically significant lower rate of progression in the ivermectin-treated group (1% vs. 22%, *P* < 0.001), with no deaths and 4 deaths, respectively. Groups 3 and 4 included only severely ill patients, with group 3 again treated with a single dose of 0.4 mg/kg plus SOC, whereas group 4 received hydroxychloroquine plus SOC. In this severely ill subgroup, the differences in outcomes were even larger, with lower rates of progression 4% versus 30% and mortality 2% versus 20% (*P* < 0.001). Ivermectin in post-COVID-19 syndrome Increasing reports of persistent, vexing, and even disabling symptoms after recovery from acute COVID-19 have been reported and that many have termed the condition as “Long COVID” and patients as “long haulers,” estimated to occur in approximately 10%–30% of cases.71–73 Generally considered as a postviral syndrome consisting of a chronic and sometimes disabling constellation of symptoms which include, in order, fatigue, shortness of breath, joint pains, and chest pain. Many patients describe their most disabling symptom as impaired memory and concentration, often with extreme fatigue, described as “brain fog,” and is highly suggestive of the condition myalgic encephalomyelitis/chronic fatigue syndrome, a condition well reported to begin after viral infections, in particular with Epstein–Barr virus. My father was on warfarin for a mechanical heart valve and even though he checked his INR monthly as recommended, it did not end well for him- INR grossly elevated resulting in brain stem bleed and death. It's not that you don't clot, it's that clotting is reduced. I suppose different medications act differently but at least with the Prasugrel that I use I just bleed a little more when I get cut. Both steel and sawmills have been reducing for the last few years. Lumber production has been in decline for 10-15 years. It's a big deal to restart a mill, in some cases the equipment and employees just don't exist anymore. The Amish built a small sawmill in Fort Fairfield, Maine last year (or the year before, I forget). And they're willing to take on jobs the big mills won't or can't. As you say they're geared for efficiency so they only take trees of a certain size, I think 12-18" at the butt or something like that.When we had the logging done on our place they cut some huge spruce trees that marked the line. Those trees had been planted as a WPA project in the '30s and were around 36" at the butt. The big mills wouldn't take them, the Amish fell all over themselves to get them.
