I think there must have been some failure at the hydraulic compensator / valve. Perhaps the hydraulic compensator jammed and broke apart, and that jammed up the camshaft so that the camshaft gear broke apart. If pistons touched valves, aren't those piston's associated bearings now also suspect?

Check the vacuum pump, if it sheds chunks it usually breaks the chain, but if the cam gear was bad, a suddenly locked chain could fracture it. Knocking before failure sounds more like a cracked camshaft sprocket though. I don't think this is a common failure Peter

No the pin was definitely there when I remove the brake assembly and put the drum/wheel back on. I suppose maybe it could have fallen out maybe? Just a quick look appears the spindle has to be cut off and new one welded back on. In other words probably the whole axle will need to be replaced
