I think your
etc/tcprules.d/qmail-smtpd is plain
text file, it should be in cdb format.
Try this to make it cdb
format...
tcprules /etc/tcprules.d/qmail-smtpd.cdb
/etc/tcprules.d/qmail-smtpd.tmp <
/etc/tcprules.d/qmail-smtpd

start up file should be look like this..
tcpserver
