I'm noticing on my live email server (hosting about 100 accounts) that
most (if not all) emails are taking anywhere from 15min to 12hr to make
it into the Inboxes.
What is strange is if I tail the /var/log/qmail/smtp/current, it
shows the email entering the system almost immediately

I had the same problem and running the queue repair did the trick.
Maybe someone else can chime in on procedure, but I think I just stopped
everything ran the queue repair and restarted everything and viola'

And for your comment hardware problem, the computer doen't freze only the
qmail don't send and receive.
