I upgrade RB 3.0.15 to 3.0.16 still have the same issue if I refresh after
adding entries to SUMMARY and DESCRIPTION field and wait for saving and
refresh and field are empty. Same is with file upload. I can get the
entries back if I do Ctrl +F5.

Thanks for coming back to me. I don't see any error in Apache or
reviewboard logs.
It is when I upload a file to newly created review entries on field,
SUMMARY and DESCRIPTION vanishes. I can get back the entry if I press CTRL
+ F5.

I did a fresh install of RB 3.0.15 with no data on it and reproduce the
same issue.
is it a bug in RB or something with APACHE as I cannot find any log
event even if I do debug.
Our old RB 2.5.16 don't show this behaviour.
