Maternal & Neonatal Sepsis Congress
The Global Sepsis Alliance and the World Health Organization have joined forces 
to host the World Sepsis Congress (WSC) Spotlight: Maternal and Neonatal Sepsis 
on September 12, 2017.

The WSC Spotlight is a free online congress in which 25 renowned experts from 
all around the world will give presentations on all aspects of maternal and 
neonatal sepsis, as well as give updates on sepsis in general. The congress 
will be held in English and is open to everyone with an internet connection.

[Sepsis Groups] Lactic Acid result/time

Good morning! Within our BayCare Health System, we use, per the Truven/CMS 
Specs guidelines, the date and time the LA was collected, which coincides with 
the result time in the Cerner EMR.  The result must be greater than 2.0 for 
severe sepsis.  Oddly, for septic shock, it must be greater than OR equal to 
4.0.  Hope this is helpful!

We have been utilizing our Sepsis Protocol for the last Month and have been 
screening and intervening on several patients. There is one Level that we need 
clarification for. Our Protocol says any Lactate Level ?2 is considered to 
indicate sepsis, but our lab range is Normal for 0.4- 2.0. Would like to know

After meeting with HIM and coding yesterday it was brought to my attention that 
the Lab normal range high is 2. Our screening form had only the > 4 mmol/L 
under #3. Criteria for Severe Sepsis. So I went back to the original form the 
actual value is supposed to be >2 . The edited form had the wrong  value of >4. 
So I have made that change on the screening form as indicated. I also, have a 
question out to the Sepsis Groups to understand how other facilities are using 
this measurement. Please make copies for your dept. and make changes to your 
existing forms thanks. Pam

Here is what we follow per the CMS standards:
Lactate greater than (not equal to) 2.0 and up to 4.0 indicates severe 
sepsis--> Lactate of 2.0 or less does not indicate severe sepsis.
Lactate equal to or greater than 4.0 indicates septic shock.

I don't know of any source for reimbursement based on the sepsis core measures 
compliance. It isn't tied to value based purchasing yet.

what they are-but the threshold for payment? Is it just SEP-1 or is each 
element measured for compliance & payment?  What percentage is required?   I 
thought it might be in the 5.3 version-but cannot find the specifics for 
reimbursement.  Any information you can share would be appreciated.  Our 
compliance rate is just over 50% & am planning on next steps to improve our

We edited our lab ranges when the measure started so anything from 2-4 is 
called an "action value", the lab still calls to alert the staff of the 
elevation.
We have an auto-reflex in place so anything greater than 2  an order 
automatically is sent so lab knows to re-draw in 4 hours.
Greater than 4 is our critical value.

[Sepsis Groups] Provider orders or protocols for IVF administered by EMS

Has anyone figured out a way to include IVF given by EMS? We are struggling 
with this since provider orders or protocols must be
found in the hospital record and none of the local EMS companies provide this 
type of information.

The NYSDOH approved protocols used by all EMS agencies in upstate NY are

There are no patient specific orders, these are standing order for all patients 
with suspected septic shock.

Has anyone found a way to comply with provider orders in the hospital record 
for IVF given by EMS?

A lactate greater than 2 could indicate sepsis if the other parameters are also 
met (suspected infection and 2/4 SIRS).
