Lenovo IdeaPad 720s (i5-8250U), the same issue.
On resume, I get:
[ 213.347656] usb 1-7: device descriptor read/64, error -110
➜ ~ lsusb -t
/: Bus 02.Port 1: Dev 1, Class=root_hub, Driver=xhci_hcd/6p, 5000M
|__ Port 2: Dev 2, If 0, Class=Mass Storage, Driver=usb-storage, 5000M
/: Bus
OK, still saw it at least once after applying 'pci=nomsi iommu=soft'.
But in general, it is much much better ;)

Title:
USB stops working after
@s-breedveld thanks. I'm absolutely sure as:
➜ ~ cat /proc/cmdline
BOOT_IMAGE=/boot/vmlinuz-4.15.0-22-generic
root=UUID=69ba76e3-5381-4011-99c1-195ad0e6d288 ro quiet splash pci=nomsi
iommu=soft vt.handoff=1
I'll try intel_iommu=off. As I said, Intel i5-8250U.
