I can add some more feedback here:

1) Identical machine, same Ubuntu fresh installed from the same media CD, but 
with a PS2 mouse instead of a USB mouse, is not suffering this.
2) Randomly happening with the machine online (even spare). I get this logged 
to dmesg, like if the mouse were being disconnected/connected. Here only 2 of 
those instances:

The device number gets increased every time.

3) Once the mouse has crashed, and without unplugging it, I have plugged
an identical mouse to a different USB port. And this one worked
perfectly. The only difference is that the port I plugged in the new one
was 3.0, while the crashed one is 2.0.

Will test now by having my crashed mouse on a 3.0 USB from boot time -
If it doesn't crash, then this problem may be affecting only USB 2.0.

both Unity and KDE.

It happens to me every 3/4 hours, so I have to reboot at least twice a
day. This is quite annoying.

As commented above, I have also tried restarting lightdm, and even rmmod
and modprobe psmouse, but it doesn't help.

Will be happy to provide feedback next time it happens if I'm given any
diagnostic commands that could help.

I get the same problem. Random mouse lock ups, the keyboard satys responsive. 
Not every day, but enough to be annoying. Loging out doesn't help, neither does 
killing off Lightdm and restarting it. 
Only fix I have found so far is a reboot.

I use a USB keyboard with an adaptor to the PS/2 port.
Fresh install
UI Unity
Linux 3.8.0-19-generic #30-Ubuntu SMP Wed May 1 16:35:23 UTC 2013 x86_64 x86_64 
x86_64 GNU/Linux
Video : Catalyst (but had same trouble with stock driver)
Architecture: x86_64
