It happened to me when I tried doing a regular text to my son, not using Siri.
Everything worked fine, even got the smiling with sunglasses emodicon or
whatever you call the thing, but I never could get it sent.

When you ask Siri, you can’t modify or add text. For this case, you should use
dictate to be able to do it.

contacts hasn't always worked. I'd also be interested in answers.
