This is correct. An Apple ID is a unique identity. Things are messy
when someone wants to mix and match app purchases and downloads with
different Apple IDs. Same goes for changing an Apple ID without
wiping the device and setting it up as a new iPhone. Just because

Yes it can. This is how I created my Apple ID when I got my iPhone. I'm
not sure since I've done it a while ago, and I already have an Apple ID
set up on my phone, but I'd take a look at Settings -> iTunes and App
Stores.
