I would put it in recovery mode, then restore it. This will not require it to
be out of iCloud, and it will erase all data if you restore as new.

I'll likely be trading in my iPhone 5 *and* iPhone 4 tomorrow for the 6S Plus.
As part of that, I need to erase them and remove them from Find my iPhone, so
they don't appear and Activation Lock can be disabled. The 5 is fine, it's the
4 that's the problem.
