Seems the bug has been fixed in the code and that the beta Apaches were still having an old copy of the code. That was most probably related to GlusterFS being stalled somehow. I have restarted the apaches instances that were showing a lot of network usage (related to gluster). A few notes from IRC:
Let me explain more:
The main entry point should be convertTo(), with convert() as a shortcut to use
the preferred (= automatically guessed from request) variant. It accepts an
almost-parsed HTML document (string) with -{}- markups embedded.
convertTo() is just a loader. It calls recursiveConvert* afterwards, which
parse -{}- syntax, and break text into pieces based on -{}- markups. These
pieces are fed into autoConvert().
autoConvert() extracts text snippets which actually need conversion (with HTML
tags, code blocks etc. excluded, but include title attribs in HTML tags
again...), then send these snippets to translate().
translate() transforms text finally using strtr()-like mechanism.

This is particularly troubling given the false positives rate of the notifications, but would be a bug in any case. Not sure I would agree. The many dialects of English, for example, can be dramatically different. 'Schedule' springs to mind[1]. Although I'd love to get into a discussion about collecting metadata with recordings (geoip location of author, self-identity of dialectic origins, etc.) I think at this point we should focus on the basic mechanics: user button to record a brief audio snippet which is auto-uploaded to commons with authoring/license templates, and the local wiktionary page updated.
