OK, re-ran it with the debug option. The output is in the same directory on my
server as the other pics - called, not too surprisingly, yos4.png ;)

Do you think it could be due to the virtual hard drive - maybe needs
formatting/dividing up? I will look into that tomorrow

On the agree/disagree section, I would change the wording to "I agree..."
rather than "I do agree..." The "I
