Good evening, I am developing a sepsis protocol for my organization as a DNP project. In looking at organ dysfunction, I have not found research that supports a time frame in which it is acceptable to use to answer the questions. To be specific, the protocol starts with the sepsis screen- the SIRS criteria. If the patient meets two or more criteria, then the nurse must address the organ dysfunction- in this section it asks to look at specific lab work. My question is- what is the time frame for the lab work-what if the nurse is completing the screen at the beginning of her/his shift at 3pm and lab work was drawn that morning? What if the lab work was drawn the prior day? I recognize that the individual lab work with elevate at different intervals- the WBC might rise slower than lactate or creatinine. Any suggestions? At what time frame should the nurse ask a physician for orders, or if protocol dictates- automatically draw the lab work to accurately answer the organ dysfunction question.
