I would like a copy of the report card too. We currently have 2 physician champions that review my daily audits of severe sepsis/septic shock patients in the ED and when a fall out occurs the physician of record receives an email with supporting data as to the corrective action needed in the future.

Let me throw out some thoughts. I once wrote this into a grant, but it was not funded. I will send you the straw man 'report cards' I made. I gave them another name. It would be great if you or someone could provide an example report card or what they believe should go in the report card. The big challenge is how do you get the data? How much is autogernated via EMR analytics. How much is collected by manual review. And how is it linked to doctors? It would seem that there are more than one doctor responsible for the data and it would also seem that nurse are as much or even more involved then the MD's. Anyway, I am really interested in discussing this offline. I am working on resubmitting my rejected grant. Please contact me directly in interest.
