The way I interpret:

Either the Lactate has to be equal or greater than 4 or there has to be 
documentation of septic shock or after the patient received sufficient fluids 
30ml/kg there has to be two consecutive BP<90 MAP<65 within hour after 
sufficient fluids. This all needs to occur within 6 hours severe sepsis time. 
If the patient did not receive sufficient fluid within 6 hours of severe sepsis 
time you would answer no to septic shock unless the patient met criteria by 
lactate or documentation....
