Hope someone can clarify this for me.
We are discussion at about the timing of the  documentation of a source of 
infection/ or documented infection.
Does the source or suspected/likely source of infection need to be documented 
before the administration of an antibiotic? And if it is not will this cause 
the measure to fail?
