The only thing related  to antibiotic administration in general are that blood 
cultures are drawn prior to administration (or there is an acceptable reason 
for not getting cultures first per the specs) and that they are started no 
later than 3 hours past time of presentation for Severe Sepsis. Documentation 
of infection time won't affect this element of the measure.

Hope someone can clarify this for me.
We are discussion at about the timing of the  documentation of a source of 
infection/ or documented infection.
Does the source or suspected/likely source of infection need to be documented 
before the administration of an antibiotic? And if it is not will this cause 
the measure to fail?
