We give Zosyn here over 4 hours as well but we have in our sepsis order set to 
give the first dose over 30 minutes and make that a STAT for our sepsis 
patients.  We also have made that order on our Rapid Response Order Sheets to 
give this dose stat and over 30  minutes.  We also try to educate that we 
always want to give Zosyn prior to Vanc.  Hope this helps.

Does anyone have a “guide” for nurses  as to which antibiotic is administered 
first.
If so, would you be willing to share?
For example, struggling sometimes with Zosyn, nurses are waiting to hang it 
last since it infuses over 4 hours.
