We first started with education one to one with the nurses.  When we would see 
someone give Vanco first we would go right to the ED and explain why any other 
antibiotic needed to be given first.  They got the hang of it pretty quickly 
thank goodness.  

Then our Sepsis Coordinator started a contest for fastest "order to antibiotic 
admin time" for the nurses.  This went over well and she gave them movie 
tickets or made certificates of achievement for them to hang in the break room. 
 It was fun and improved our abx admin times tremendously!

Our pharmacy also calls the ED 15 minutes after a Code Sepsis is called and 
speaks with the provider about abx just to keep the ball rolling.

We give Zosyn here over 4 hours as well but we have in our sepsis order set to 
give the first dose over 30 minutes and make that a STAT for our sepsis 
patients.  We also have made that order on our Rapid Response Order Sheets to 
give this dose stat and over 30  minutes.  We also try to educate that we 
always want to give Zosyn prior to Vanc.  Hope this helps.

Zoysn can be infused in 30 minutes
Vancomycin or quinolones such as Levaquin or cipro take 120 minutes..

If your providers are using both antibiotics ( our staff does).. we have 
educated our nurses Fastest First.   This little reminder has helped reduce the 
number of fall outs related to antibiotics  started timely for the SEP-1 bundle

Here is a draft of one I am working on. My nurses do not always have enough 
pumps to do concurrent administration, so I am pretty sure that we will go to 
infusing just the first dose over 30 minutes instead of 3-4 hours.
