Could the proxy server be caching requests?  I.E., if the proxy server
gets a request like "http://blah.blah/something/page.jsp", maybe it's
returning the same page that it did before when it got that request, even if
it's coming from a different user/session.

Just a thought...

Huh? What sort of proxy are you talking about here? An
HTTP proxy?

Sounds to me like a broken/misconfigured proxy.

As far as the origin server is concerned (the one behind
the firewall) a request from the proxy server should
look almost exactly the same as a direct request from
the client. Under normal circumstances the only
differences will be that the remote IP address will be
that of the proxy rather than the client and the proxy
will have added a Via: header. So, no, servlets won't
normally be aware of the presence of a proxy (unless
they examine the Via: header or the remote address).

Of course, your firewall may be doing things to the
traffic as well, but that's going to be very site
specific.

I suggest you have a chat with your sysadmin.
