What is the best way to approach this Orchestrator or SMA. Any advantage one 
over the other?

Use the Monitor Object activity at the beginning of a SCO runbook.  It will 
continuously poll the SCSM DB (every 30 seconds I believe).  You can look for 
manual activity objects that have the notes property value change.

Hi Travis,

In orchestrator would I monitor an SCSM object? If so how would I make sure 
that it keep monitoring once the runbook has ran once?

Only by using something like Orchestrator or SMA to traverse the relationships 
to figure out who to notify.

added to a manual activity to notify the Incident or Service Request Owner?
