Sounds like you've lost a shift fork and/or damaged 5th gear somehow. Luckily, 5th is at the end of the transmission behind the cover under the driver's side wheel well.
Remove the cover by draining the fluid, pulling the plastic shields out of the way and remove the bolts holding the cover in place. If the gear/syncro/fork looks damaged or pieces come out, then it's time to do a rebuild. If the fork is all that is damaged then you be able to replace it in place. Refer to the FSM's for information on transaxle repair. One of the springs built into the shifter is for the reverse lockout I believe. The shifter should also have a small spring, for the side to side movement of the shifter, built into the lever arm the shifter actuates to transfer the side to side movement into the fore/aft needed for the cable.
