That brings to mind Frank Herbert's best seller "The White Plague".
The protagonist's wife and children are killed during an IRA attack, so he
decides to take revenge on the entire country of Ireland.
He develops a plague that only kills women (very quickly).
Only he has the antidote.
But his method of spreading the plague always struck me as very plausible:
he purchases things from Irish merchants and pays with cash that he has
infected.
Cash, of course, moves around a lot and would spread a plague very
effectively.
Ever since reading that, I hate handling paper money.
Good reason to use credit/debit cards - or return to gold and silver coins
(harder to infect?).

Like you said, accidents happen.
But if starting a pandemic is what you WANT to do, isn't it a lot easier
and more effective to infect a traveler without his knowledge and no way
to track where it came from, with a big delay in finding out what it is?
..preferably a hitch hiker without access to medical help that comes in
close contact with diverse elements over a wide area.
