On PSTN also there are mechanisms to charge based on bearer capability
(module code 071 which captures info from  Generic Base USI from ISUP
signalling , as specified by BAF GR-1100-CORE) , So you can be charged more
if you use more bandwidth.  I think you should be charged more if you
initiate a video session and less if you initiate a audio session for the
same session duration. Also if you request specifically for higher QoS ,
then you should be charged extra for that? for example , if you have
subscribed for a better QoS and your voice packets are routed over a
expensive ATM backbone, then you should be charged more than a simple best
effort call.

 I am not sure how a 3rd party would collect RTCP reports, given that the
RTCP reports are sent end to end.  To do this means that you actually have
access to the end users.
While RTCP reports will provide additional granularity, and like you said,
provide feedback on the QoS, would it not be adequate to use the PSTN model,
and charge for connection time?  I dont usually get a lower rate for a line
with static :).
