One thing I see is that your BYE request doesn't contain a Max-Forwards header, which is required per section 8.1.1 of RFC 3261.

When I use another remote UA (josua -- http://www.fsf.org/software/osip/) the BYE gets a positive response (200 OK).
Is there something wrong with the following message:
