am not exactly sure if it matters, whether the Cisco GW is configured such that calls arriving at a given port are forwarded to the SIP phone directly (i.e. the session target = ip address of the SIP phone) or to the the sip registrar. By changing these values i am getting different responses

As I understand you want SNOM to transfer POTS call to KPHONE? If you are initiating call transfer from POTS, Cisco gw maybe performing blind transfer instead of attended call transfer. I am not sure if there is any configuration on Cisco which will allow you to do attended transfer.

Or, if initiating from SNOM, it can be an issue with SNOM doing blind transfer. In 
this case you will have to check SNOM onfiguration.

I have the following call:
SIP(SNOM) --> PSTN GW (CISCO) --> POTS destionation

I am asking the SNOM phone to issue a transfer to another SIP Phone (KPHONE), however i am loosing the connection with the POTS destination before i can establish a VOIP connection with the KPHONE. Now, is there a way to configure the CISCO box to swap the SIP leg of the SNOM phone with the newly established KPHONE leg without loosing the POTS leg?
