   Indeed, I don't see any real reason we should not move toward assuming
   that all requests are handled the same way.

Of course, the proxy doesn't have to keep state for any dialog.  I
suppose we want to circumscribe the number of things the UA has to
keep state for.  But it is sort of ironic that an incoming request can
easily carry enough information to provide for a reliable reverse
communication path, and yet we can only exploit that by creating a
dialog for it, which is too heavy-weight.
