AFAIK the only "real" solution (in SIP UDP) is the client or the server to 
mantain the UDP "connection" open by sending OPTIONS.

The problem is that many times a UAC just sends OPTIONS if working with STUN 
and a server just sends OPTIONS if it detects the client as behind NAT (but 
with ACL there is no STUN and server can't imagine if client is behind NAT or 
not).
