strong consensus was reached at IETF 73 in minneapolis for adopting:

to solve this very problem.

right now, it's a mess - everyone does there own thing.  most rely on
To, which is horribly broken (see [1]).  Some use P-Called-Party-Id,
some use route headers.

I'd recommend adding the History-Info target parameter solution,
although bear in mind it's still in draft, so caveat emptor.
