OK. Looking at it again, the flows you describe aren't consistent with 
the transfer you seem to be talking about.

If C called A, and then A wants to transfer the call to B, then the goal

is to have C talking to B, right? That's not what you showed.

And the simple answer is that you only have two choices:

- send a REFER to C, or B, to directly connect them.
   But I think you state the SS refuses this.
   (Depending on signaling, you *might* be able to
   bypass SS. But I doubt it.)

- you must insert something, which will have to be a B2BUA,
   to bridge the B call leg to the C call leg. It could be
   A, but the PBX is a more likely candidate.
   But you might not have to relay the media. With some care
   and some luck, you might be able to offer B's sdp to C
   and C's to B. Then you would be in the signaling path, but
   not the media path. That would make it a fairly small cost.
