Yes registrar must support GRUU. If it supports it will mention 'gruu' 
extension in the Supported header.

If any UA supports GRUU it will put +sip.instance and corresponding 
pub-gruu or temp-gruu in its contact header.
Other UAs use this gruu to route requests/responses.
