yes we do have that, but if all UAs would register with all proxies,
there would be no need for a shared database which would greatly
simplify the implementation.  by the way, there is no good open source
shared database system available today.

it is not just about shared database, since if the UA is behind nat,
the other proxy is not able to reach the UA.  and if the proxy the ua
registered with is down, then ...
