[JRE] Exactly. But viewing it my way possibly requires less change to
sip-outbound, since much of it talks about one instance ID per UA (e.g.,
section 3.1).

Whichever way we view it, the following statement in 4.1 seems to be
problematic:
"Instance ID MUST NOT change as the device moves from one network to
   another."
