Good intuition.   It did end up being the line after the [last_to]  
wasn't matched.

Below I gave the wrong xml snippet.  What my file had after the  
[last_to] was:
        [route]
in order to match any routing headers.

Isn't this a bug?  If there are no route headers,  don't generate  
anything!

Is there some workaround where if there are route-headers in the  
request, they will be generated in the response... and if there are  
no route-headers,  then don't give the '\r\n'?
