There could be a use case for this ( not sure how compelling it is).
Consider that you are making a call from a SIP video phone via an ITSP
to a second PBX where the DID of the inbound call is mapped to another
video phone.

It would be nice if this case could work in a future release.

For now, I will deal with a single voice sdp media description per
call -- which should cover all PSTN interactions.
