Mircea A. decided to go with the MAC address.
So right now, if the correct mac address is not provided in the interface, provisioning won't work.

2. Even if you put the proper mac address into sipXconfig the Client asks for 3 parameters during provisioning.

    * User
    * Password
    * Server IP

I am guessing the user/password is for the account I want to provision and the server ip is for the sipXconfig box? None of these work.

You're right, user/password are the ones for the first account (first line).
The problem is with the Server. This should point to the new login server, which should be something like: http://YOURHOST:12000/cmcprov/login According to the Counterpath Manual, this can be provided by the DHCP server.
