I've noticed that when the Emergency Dial Rule is enabled, sipXconfig
generates Polycom profiles that cause INVITEs for emergency calls to be
sent directly to the gateway.  The new Alarm notification behaviour is
then not triggered on emergency calls, because the request was not sent
via sipXproxy.

This seemed like a bug, but Carolyn pointed me to help text on Emergency
Dial Rule screen.  The text rambles quite a bit, but does seems to
indicate that this is intentional.  Polycom SoundPoint IP, LG-Nortel
68xx, and Cisco are listed as the only phones that this applies to.

I think we should remove this behaviour, since it prevents the new Alarm
notification from being triggered by Polycom emergency calls.  

(The problem doesn't show up on LG-Nortel 68xx phones, due to XCF-3575:
Invalid LG-Nortel config file generated when Emergency Dial Rule is
enabled.)

A superadmin that wants phones to send emergency INVITEs directly to the
gateway would then explicitly configure this using a Phone Group.  This
can be done under Lines -> Dial Plan -> Emergency Gateway for Polycoms,
and under Dialing (emergency_...) for LG-Nortels.

Any objections to an Improvement JIRA for this?  Thanks.
