Looks good. Watch the labels though: they should have sentence capitalization.
"Include audio as attachement" and "Attach voicemail" would be better as
"Attach audio".

Why do we have E-mail format options only for primary e-mail address?
There should be a way of having alternate e-mail address without v-mail
forwarding enabled. I don't think UI you proposed allows for that.

So if we are synchronizing with IMAP there is no control over e-mail format
or audio attachment, right? Is that a feature limitation or is that
something that we are choosing to enforce?

Why would you remove "Voicemail attachment options" from user portal? Admin
can configure IMAP server but users have to be able to decide how they want
to sync/forward their voicemail notification. If IMAP server is not
configured users obviously should not be able to select IMAPsync. But if it
is configured why force them? Especially since it's not working unless they
configure a password.

What kind of provisioning scenario we want to support with this UI. I think
the only workable solution is that admin configures IMAP servers (most
probably on a group level) and then users choose (or not) to sync. What
other scenarios people have in mind?

On a related note. Do we have a wiki page that would describe advantages of
synchronizing voicemail with IMAP vs. just forwarding voicemail
notifications. It would be a good place to maintain a list of IMAP servers
on which this feature actually works and some explanation on how using it
limits the ability to change e-mail address.
